Trigger Points
Have you ever had a muscle ache so bad that you could barely stand to touch it? Do you have a sore spot where the smallest pressure can send shooting pains down your arm, leg, back, etc? If so, you might have a trigger point. Anyone who has had a trigger point can tell you that its like an "x marks the spot" for pain. If you have one and someone presses on just the right place, you'll know!
What is a trigger point?
A trigger point is a spot in your muscle that has become "hyperirritable." This means that the muscle is sore and inflamed, and that pressure on that spot can cause pain to travel away from the original spot. Pain from trigger points is usually described as deep and aching and is often constant. You may see people with trigger points trying to massage their own muscles to get relief. Trigger points can sometimes be so uncomfortable that they can prevent you from being able to carry out your day-to-day activities.
How do trigger points form?
Your muscles are controlled by nerves that carry messages to and from your brain. When you have a tight muscle or an injury, the nerve going to that muscle is getting lots of extra information and can become irritated. The irritated nerve causes more tightness and pain in the muscle. Do you see the pattern? A trigger point is kind of like a snowball rolling downhill…once it gets going it can be hard to break the cycle without medical attention.

How are trigger points treated?
There are a number of different ways to treat trigger points. The treatment that is best for you will depend on how bad your symptoms are. Mild trigger points may resolve on their own with rest or something as simple as a hot shower. Moderate to severe trigger points are likely to require treatment from a medical professional.
When trigger points first form, they may improve with some simple treatments that you can try at home. Ice packs or heating pads can be enough to give some pain relief and "break the cycle" that was described earlier. If you don't have an ice pack or a heating pad, a bag of frozen peas or a nice hot shower are good substitutes. The next thing to try at home is rest. You have to give your muscle a chance to recover. For example: if you have a trigger point in your shoulder, its probably not a good time to be painting the house or starting another project that requires a lot of work from your shoulder muscles. Allowing yourself a little time to heal might keep you from getting worse and the paintbrushes will still be there another day! Another home treatment that can help trigger points is a good stretching program. Remember from before that trigger points can form in tight muscles. By improving your muscle's flexibility you can help the trigger point by taking away the underlying cause, or what started the problem in the first place. If you've never done stretching on your own, a physical therapist can help you learn the correct techniques to get you the most benefit.
If you've tried these treatments and you're still having a problem, then you should consider seeking medical attention and the sooner the better. Trigger points that are ignored for too long can become chronic and much more difficult to improve. Also, if you have more than one trigger point, you may have a condition known as myofascial pain syndrome, which involves general achiness on top of trigger points. Many doctors will recommend that you try physical therapy as the first treatment for a trigger point. A physical therapist can use some of the treatments described above along with hands-on treatments like acupressure and myofascial release (a sort of deep tissue massage) to help relieve your pain. After your pain is improved, the PT can help you to correct problems that cause trigger points to form in the first place, such as weakness, poor flexibility, or poor posture. Treatment of these issues is very important in order to prevent your pain from coming back in the future, so its important that you stick with your treatment plan even when you start to feel better. Remember that the goal in physical therapy is more than just helping you feel better right now, its also preventing you from feeling lousy again in the future!
Some people who get trigger points will tend to get them over and over again until they address the underlying problem. A chiropractic evaluation can help to determine if a problem with the structural alignment of your body might be contributing to the problem.
There are some cases when a stubborn trigger point needs further attention from your medical doctor. If conservative treatment and physical therapy are not successful, your doctor can explore other options with you. Treatments such as acupuncture, medication, and TENS may help to improve your pain.
